Nursery 1, Nursery 2, and Kinder

  1. PSA birth certificate
  2. 2×2 most recent picture with white background (upload .jpg file)

Grade 1 to Grade 11

  1. PSA birth certificate
  2. Report card of last completed grade level
  3. Latest grades of the current grade level. Upon enrollment, the original and final report card MUST be submitted to the Office of the Registrar.

Requirements

(For Filipino Citizens studying abroad)

Nursery 1 to Grade 11

  1. PSA birth certificate/birth certificate from abroad
    * If born abroad in a non-English speaking country, have the birth certificate translated in English and duly authenticated by the Philippine Embassy/Apostille Stamped located in the student-applicant’s country of origin or legal residence, both in English and in his national alphabet.
  2. Passport of both parents
  3. If the applicant has dual citizenship, submit a photocopy of Philippine passport and/or Certificate of Recognition as a Filipino citizen
  4. Complete school records of all grade levels attended and duly signed by the school principal (From Kinder to last grade level attended).
    * For non-English speaking countries, have the school records translated in English and duly authenticated by the Philippine embassy/apostille stamped located in the student-applicant’s country of origin or legal residence.
  5. Certificate of Enrollment of the current school year or Certificate of Completion of last completed school year
  6. School profile of the current school
  7. School accreditation from the education agency where school is located
  8. 2×2 most recent picture with white background

Nursery 1 to Grade 11

  1. Passport biodata page
  2. Valid visa
  3. Birth certificate from abroad/PSA birth certificate
    * If born abroad, in a non-English-speaking country, have the birth certificate translated in English and duly authenticated by the Philippine Embassy/apostille stamped located in the student-applicant’s country of origin or legal residence, both in English and in his national alphabet
  4. Passport of both parents
  5. Alien Certificate of Registration (ACR) / I-CARD (Photocopy of front and back portion), if available
  6. SPECIAL STUDY PERMIT (SSP) for a foreign student enrolled in the Philippines of the current school year, if available
  7. Complete school records of all grade levels attended and duly signed by the school principal (from Kinder to last grade level attended).
